Support Students to Prepare for What Comes Next

Schools are often the first place families turn to when they start thinking about life after school. Transition conversations that begin early and are backed by good information, make a real difference for students and their families.

Endeavour Foundation can support your school with practical information about post-school pathways across work, community participation, skill-building and future support options. Our goal is to give your team the knowledge and resources to have confident, well-informed conversations with students and families.

Whether you are preparing for a transition planning meeting, looking for resources to share with families or exploring what pathway options are available locally, we are here to help.

How We Can Support Your School

We know that educators need practical, credible information they can use in the classroom, in transition planning conversations and when talking with families.

Here is how we can help:

  • Pathway information about work, community participation, skill-building and future support options
  • Resources designed to be shared with students, parents and carers
  • Local service information across our Queensland network and beyond
  • Conversations with your transition planning team about what is available locally
  • Information sessions and school engagement where available and local capacity allows

We are not here to replace the work your school does - we are here to give your team better information, so students and families feel more prepared and confident about what comes next.

Need Post School Transition information for your school?

We can provide resources, local service information and guidance to support classroom learning, transition planning and family conversations.

Getting Ready for the Transition From School

It is never too early to start thinking about what comes next. Years 10 to 12 are a great time to begin exploring - even if your young person is not ready to make decisions yet.

Early conversations can help your family feel more confident about the options ahead. You might start thinking about:

  • Work interests and future goals
  • NDIS funding and support needs
  • Community participation and social connection
  • Daily living skills and building independence
  • Transport and routines
  • Local support and employment opportunities

You do not need to have all the answers before reaching out. That is exactly what our team is here for.

When to Start Post-School Planning

Research and experience consistently show that the earlier transition conversations start, the better prepared students and families feel.

Years 10 to 12 are the ideal window for schools to begin exploring goals, interests, NDIS funding needs and local pathway options with students and their families. Early conversations help reduce anxiety, build realistic expectations and give families time to make considered decisions.

Students who are explored transition options early - even in a low-pressure, exploratory way - tend to feel more confident and prepared when the time comes to act.

Even if a student is not ready to commit to a direction, access to good information early makes a genuine difference.

Frequently Asked Questions (FAQs)

Yes. You do not need to have a specific student referral ready. Schools can contact us for information about post-school pathways, local service options, resources for families or support with transition planning conversations.

Students may explore pathways across community participation, skill-building, employment, work readiness, Supported Employment, inclusive employment and future home and living options.

The right pathway will depend on the student’s goals, interests, location, support needs and NDIS funding.

Students may need NDIS funding to access some supports, such as community participation or learning and lifestyle programs.

Some employment pathways may not require NDIS funding, depending on the student’s goals, eligibility and the type of support they are exploring.

Where available, Endeavour Foundation may be able to support school engagement activities, information sessions, transition events or local pathway conversations. Availability will depend on location, timing and local service readiness.

Yes - if a student and their family are ready to explore specific services, our team can guide you to the right enquiry pathway. However, you do not need to have a specific student referral in mind to get in touch. Schools can contact us for general pathway information, transition resources and school engagement opportunities at any stage.

This is common. Endeavour Foundation can help explain different pathways and what may be suitable based on the student’s goals, support needs, funding and location.

It is helpful to start conversations before a student’s final year of school. Years 10 to 12 are a good time to explore goals, interests, support needs, NDIS funding and local service options.

Supported Employment is employment at an Endeavour Foundation social enterprise, where people with disability work alongside other employees with disability and receive ongoing on-the-job support. Open employment (also called inclusive employment) means working in a mainstream workplace, with support from our team to find the right role and employer. Both are valuable pathways - the right fit depends on the student's goals, confidence, readiness and support needs. We can explain both options clearly if you want to discuss a student's situation.

Yes - where available and where local capacity allows, Endeavour Foundation may be able to attend school events, transition expos, information evenings or planning meetings to share information about post-school pathway options. Availability will depend on location and timing. Get in touch with our team to discuss what may be possible for your school.

Endeavour Foundation has its largest service network in Queensland, with services gradually expanding across Australia. If you are based outside Queensland, our team can advise whether services are available in your area and help connect you with relevant information or pathways even where direct services may not yet be available.
