Supporting Stable Transitions into Adulthood

Endeavour Foundation provides specialist support for young people with intellectual disability who are preparing to transition from statutory care into adulthood.

Our approach combines disability expertise, trauma-informed care, structured transition planning and strong safeguarding to support greater stability, skill development and long-term pathways across home, community, education and employment.

Young people with intellectual disability in statutory care may be navigating changes across Child Safety, the NDIS, education, housing and adult services at the same time.

General residential care models may not always be designed around their disability, communication, behaviour support and individual functional needs.

Youth Disability Services provides a specialist pathway focused on:

  • Safe and stable support
  • Progress towards individual goals
  • Building skills for independence
  • Greater choice and participation
  • Continuity and reduced disruption
  • Connection to family, kin, culture and community
  • Education, employment and stable living pathways
  • Preparation for suitable adult supports

Youth Disability Services can also help connect young people with relevant Endeavour Foundation and broader community pathways based on their goals, funding, eligibility and individual circumstances.

Frequently Asked Questions (FAQs)

Child Safety representatives, government stakeholders, placement providers, Support Coordinators, foster and kinship carers, family members and other authorised supporters can contact Endeavour Foundation about Youth Disability Services.

Funding requirements will depend on the young person’s circumstances and the type of support being considered.

A young person may initially be supported through Child Safety arrangements while preparing for longer-term disability, community or NDIS-funded supports. Our team can discuss the current funding arrangements and what may be required for the next stage of their transition.

No. An enquiry gives our team the opportunity to understand the young person’s circumstances and explore whether Youth Disability Services or another Endeavour Foundation pathway may be suitable.

Placement and service availability will depend on the young person’s funding, location, compatibility, support needs, safeguarding requirements and current capacity.

Early transition planning is encouraged, ideally well before the young person turns 18.

Starting early provides more time to understand the young person’s goals, review NDIS and other funding arrangements, clarify responsibilities, explore suitable adult supports and prepare them for change.

No, Endeavour Foundation can support broader pathways based on the young person’s goals, funding and needs. These include:

  • Daily living and independence skills
  • Community participation
  • Education and employment pathways
  • Home and living supports
  • Connection with adult disability, NDIS and community services

Youth Disability Services is one part of this pathway, and our team can help connect the young person with suitable supports.

Transition planning may involve the young person and members of their broader support network, including:

  • Foster and kinship carers
  • Parents and family
  • Child Safety representatives
  • Placement Services and Transition to Adulthood teams
  • Support Coordinators
  • Schools and transition staff
  • Health and allied health professionals
  • Behaviour support practitioners
  • Housing and disability providers
  • Culturally appropriate services and community representatives

The young person’s preferences, communication needs, identity and goals remain central to the planning process.

Yes, where this reflects the young person’s wishes and is safe and appropriate.

Important relationships with foster and kinship carers and family and cultural and community ties should be considered throughout transition planning and into adulthood.

  • Behaviour support and clinical oversight
  • Proactive risk identification and management
  • Clear escalation and reporting pathways
  • Person-centred planning
  • Workforce screening, training and supervision
  • Regular review of service quality and outcomes

This approach aims to strengthen stability, reduce avoidable disruption and create a safer pathway into adulthood.

  • Stronger daily living and independence skills
  • Greater choice and participation
  • Clearer pathways into adult disability supports
  • Stronger family, kin, cultural and community connections
  • Increased opportunities for education, community participation and employment

Endeavour Foundation has extensive experience supporting people with intellectual disability and complex support needs.

Youth Disability Services has supported young people across Brisbane and Moreton Bay, including young people who have transitioned from statutory care into adult NDIS-funded Supported Independent Living and continued to access other Endeavour Foundation services.

Our experience includes:

  • Supporting transitions from statutory care into adult disability services
  • Developing structured and sustainable living arrangements
  • Building daily living and independence skills
  • Maintaining important family, kin, cultural and community connections
  • Coordinating pathways across home, community and employment
  • Working collaboratively with government and professional stakeholders

You do not need to have every detail finalised. Where available, it helps to share:

  • The young person’s age, location and current living arrangements
  • Disability, communication and support needs
  • Funding and NDIS status
  • Transition timing and any goals
  • Important family, cultural and community connections
  • Relevant assessments or support plans

Our team will contact you to learn more about the young person and their circumstances, support needs and transition timing.

We will then explain whether Youth Disability Services or another Endeavour Foundation pathway may be suitable and outline any information or next steps required.

An initial conversation does not commit the young person or referring organisation to a service.

Service availability will depend on the location, service capacity and the young person’s individual needs.

Our team can explain which Youth Disability Services or related Endeavour Foundation pathways may be available at the time of enquiry.

Yes. Government and sector stakeholders can contact the Youth Disability Services team to discuss regional needs, transition pathways, future service opportunities and potential partnerships.
