Endeavour Foundation is coordinating the rollout of COVID-19 vaccination hubs in conjunction with the Australian Government. These hubs are for NDIS participants, their primary carers and support workers, to be vaccinated if they choose.

Toowoomba Vaccination Hub: Former L&L Herries Street.

Open to: NDIS participants, their primary carers and support workers.
This hub will be open to the broader disability sector, so if you know people outside of Endeavour Foundation who would benefit, please let them know about the hub.

What you need to bring

On the day of your vaccination, please bring:

  • For your 1st dose: Government Consent Form, your Medicare Card (essential) and Staff ID
  • For your 2nd dose: Original Consent Form from 1st dose - these forms were given back to you for safekeeping after dose 1. Please bring them back with you!

Please Note: It is essential to keep proof of your first dose and bring this to your second dose appointment.

About our Vaccination Hubs

The Australian Government has contracted Healthcare Australia as the vaccine provider. The hub is staffed with an Immunisation Nurse (dispensing vaccines), a First Aid Nurse (for observation after the vaccination), and clerical staff to enter vaccination details into the government register. An Endeavour Foundation staff member will also be on hand to assist.

After your vaccination

Your vaccination record:

If you are with Endeavour Foundation services at home, at work or in the community, we will record your vaccination.

This will help us to support you getting your second dose of the vaccine on time.

The Government will also record your vaccination in their myGov system.

Feeling unwell?

You will be monitored for at least 15 minutes after your vaccination.

You will also be given some details about possible side effects.

If you are concerned that you feel unwell after your vaccination, please contact our COVID Hotline on 1800 983 006.

We understand if you need to stay home until you are feeling better.
