Endeavour Foundation is coordinating the rollout of COVID-19 vaccination hubs in conjunction with the Australian Government. These hubs are for NDIS participants, their primary carers and support workers, to be vaccinated if they choose.

Location: 38 Railway Parade, Geebung QLD 4034 (former Learning & Lifestyle building)

Opening times 9:00 am - 4:30 pm

Vaccine offered: Pfizer (age 16 and above – no upper age limit)

What you need to do NOW so you can receive your vaccination at our hubs:

First, let us know you want to be vaccinated.

Then, complete a Consent Form, unless you have already done so.

If you have already provided a consent form, NO new form is required

Complete the form and hand to your site manager or Employment Coach as soon as possible.

If you are unsure about medical questions on the consent form, please discuss this with your doctor.

After your vaccination

Your vaccination record:

If you are with Endeavour Foundation services at home, at work or in the community, we will record your vaccination.

This will help us to support you getting your second dose of the vaccine on time.

The Government will also record your vaccination in their myGov system.

Feeling unwell?

You will be monitored for at least 15 minutes after your vaccination.

You will also be given some details about possible side effects.

If you are concerned that you feel unwell after your vaccination, please contact our COVID Hotline on 1800 983 006.

We understand if you need to stay home until you are feeling better.
