Endeavour’s individually funded services offer a range of tailored programs designed to meet each person’s interests and goals. The programs provide individuals with the opportunity to achieve independence and enhance life opportunities.

Support is provided on a one-to-one basis unless otherwise requested by the participant.

Lifestyle Support services include:
•    Drop-in support to enhance independent living options.
•    Life skills training.
•    Community access to recreational, social and educational activities.

Endeavour will help you, your family or carers to plan for your future. Often the biggest challenge is working out what you want to do.

Who is eligible?
To be eligible for Endeavour’s individually funded service, a person must:

•    Be 18 years of age
•    Have a disability as defined under the Queensland Disability Services Act 2006
     (visit www.disability.qld.gov.au)
•    Be in receipt of individualised funding
•    Be in a position to self-fund the cost

How do I apply?
To apply for an Adult Lifestyle Support package, complete a Registration of Need form and/or Family Support Program for Adults form, available from Endeavour or DSQ.
