Then you need to make choices about what you want to do next. Endeavour's Post School Services (PSS) program supports young adults with disability through education, life skills and training, integration in the community and community based activities.

With Endeavour's support, young people develop friendships, skills and knowledge about life in the community.

Endeavour's Post School Services program offers you choices. You can create your own program to suit your needs and fulfil your goals. You can access one-on-one support, group support through any of our Adult Training Support Services (day services) or a combination of both. You can also access our local Business Services, which give people the opportunity to develop their vocational skills.

Endeavour's Post School Services program has trained, qualified staff to meet the individual needs of people with intellectual disability with low, medium and high support requirements.

You are about to experience some big changes. Endeavour will help, your family and your carers to plan your future so you can make the change from student life to community life. Endeavour's Post School Program is flexible and can respond to your changing needs over the coming years.

Who is eligible?
To be eligible for Endeavour's Post School Services program, a young person must:

• Be 18 years of age
• Be finishing or have finished school
• Be eligible for assistance under the Disability Services Act 1992 (Qld) (visit www.disability.qld.gov.au for funding information)
• Not have access to tertiary education, vocational training or employment options

How do I apply?
To apply to join Endeavour's Post School Services program, complete a DSQ Registration of Need form and a Post School Services Program Application form.

Both forms are available from your local Endeavour Post School Coordinator or your School Transition Officer.
